During the therapy process, all information is handwritten and kept as hard copies. Once you have finished your therapy your notes will be securely destroyed.

Notes: Records are kept to enable your therapy, ensure your safety in the process, and to improve the quality of your treatment.

Notes: Anonymously presented at supervision to meet professional criteria and ethics. Shared with your explicit consent with other professionals should the need arise for the benefit of your therapy.
How is the information stored

Contact information: Separately from treatment notes in a locked filing cabinet

For how long is the information stored
Contact information and details as to the frequency of treatment will be stored for eight years. Any notes are securely destroyed on completion of the therapy.
Your rights

You have the right to have access to the information that I hold on you. I will respond within 40 calendar days of the request and can provide a copy of the information that I hold on you free of charge, unless the request could be deemed to be excessive.

You have the right to correct the details that I hold on you. I will correct the error within one month of the request.

You have the right to withdraw your consent regarding holding information on you, at any point. Should you withdraw your consent, all information held on you will be destroyed.

As all notes will only exist in hard copy form, should you ask for your information to be transferred, I can provide copies of your information.
